Welcome! We use this multi-purpose smart form for all our appointments to sure things don't get missed and your pets get the best care possible, so we do ask you to fill it out at each visit. However, it's SMART, so it will be VERY short if you've been in recently and don't have new concerns!

AVC offers convenient Virtual Care options for our Wellness Members, including full consultations with our doctors, Case Review for straightforward questions, and some specific convenient options to help with things like noise phobia or anxiety at the groomer.

Note: We are happy to provide complete medical records to your insurance company on request and with your approval. All records are considered complete and final; no information or paperwork beyond this will be provided to them.

IMPORTANT: New client appointments are generally unavailable for 1-2 weeks.

A non-refundable $25 deposit is required to book your first appointment. This reduces no-shows and last-minute cancellations that impact our ability to care for our patients. 

NOTE:  Both offices are only a few minutes apart and operate as one, as do our staff. During busy periods first appointments for new clients may be limited to our larger New Albany office, though once established clients may request either office if available. Additionally, surgery is performed only at the New Albany location, while ultrasound and dentistry is available at Jeffersonville.

This is typically a significant benefit for you and your pet, as it allows more timely availability and a wider range of services in a more cost-effective manner, and our offices are only minutes apart.

Contact your prior veterinarian in advance of your visit to obtain records - they may be emailed to records@accessvetcare.com, faxed to 812 590-3083, or dropped off in person. If records of vaccinations or diagnostics are not provided prior to your appointment, it may be necessary to repeat them.
